Joseph Saint Joseph, MO Saint Joseph, MO Full-time Full-time $15 - $16 an hour $15 - $16 an hour 20 hours ago 20 hours ago 20 hours ago The Community Support Worker (CSW) will optimize and coordinate services that encourages and empowers individuals and families living in poverty to overcome economic and social barriers to achieve long-term stability.
The CSW will support and coach at-risk clients to manage/navigate the health care and social service systems.
Community Support Workers (CSWs) bridge the gap between people in need and the resources that will help improve their lives.
CSWs also serve as a link between the client and the resources/tools needed to stabilize their lives.
Key Responsibilities Administration, Planning and Evaluation Is familiar with resources available to assist low-income children and families.
Conducts assessments with prospective participants to determine participation eligibility and identify social and economic barriers.
Completes enrollment forms and other documentation in a timely manner.
Assists participants with the development of goals that will improve their respective lives and ultimately help them to become self-sufficient Encourages self-sufficiency by showing the client how to access/complete needed items for each resource process.
Responds to referrals and participant needs in a timely and professional manner.
Provides regular and timely updates to referral sources on participant's progress on meeting goals.
Builds relationships with community partners and stakeholders.
Completes all required documentation and data tracking in a detailed and timely manner Maintains accurate and organized participant files and related documentation.
Attends and actively participates in trainings and meetings.
Completes all other duties as assigned.
Quality and Community Relationships Maintain good communication with program staff and community/agency businesses.
Establish and maintain high service quality to customers through sustainable systems.
Supervision and Staff Relationships There are no supervision requirements of this position.
Cooperate with other staff to ensure high quality service provision Participate in staff meetings, and other team meetings; as needed; Qualifications Ability to relate to diverse groups of people and people on all levels.
Must obtain a Missouri Class E Driver's License, reliable transportation.
Ability to prioritize and self-motivate to complete tasks.
Clear and effective written and spoken communication skills.
Knowledge of community resources Proficient with Microsoft Office Suite or related software.
Physical Requirements:
Prolonged periods of sitting at a desk and working on a computer.
Must be able to lift 20 lbs.
at times.
Education & Experience High school diploma or GED with two years experience working with low-income children and families required; Bachelor's degree in Social Work, Human Services or closely related field and two years of job related experience preferred.

The check will bounce and the victim is left responsible.
