Responsibilities:
Complete the PCS Support Coordination training program:
attend all needed training, fieldwork, and training modules.
Work in conjunction with your supervisor to ensure your training needs are met Complete the Department of Mental Health mandatory Support Coordination training within 90 days of employment.
Participate in Charting the LifeCourse training to learn about tools and philosophy Learn about and utilize exploration tools and assessments to help determine service needs, both initially and ongoing Begin coordinating and monitoring the ongoing care of a caseload, ensuring health, safety, freedom from abuse/neglect, and the ability to experience one's hopes and dreams.
Caseloads involve a wide range of disability areas and age ranges Build professional and respectful relationships with people you support, the teams surrounding them, community partners, and fellow employees o Demonstrate sincere and respectful communication (both verbal and in writing) using People First Language Work with each person you support to develop a yearly person-centered plan focusing on identifying their goals, interests, relationships, preferences, successes, and support needs Follow the planning process guidelines and complete all processes of the yearly person-centered plan within the timelines identified for each person supported.
Compile and maintain records on direct contacts, progress notes, and monthly summaries, and prepare reports concerning the individual and/or their family and progress toward ISP outcome.
Complete Individual Support Plan (ISP) amendments as needed Visit those supported in their home and/or the community regularly.
Monitor for health & safety, quality, timeliness, risk management, effectiveness, and overall service delivery Complete visits and all documentation for visits within the required timeframe (monthly, quarterly, or as directed) Link people to support and services, from requesting funding to monitoring delivered services Submit timely service requests and funding components as needed for those we support Use software systems to track, log, and maintain documentation while remaining in compliance with agency, state, and federal guidelines Use Microsoft Teams and Outlook Calendar to manage and promptly keep appointments Provide assistance in accreditation and audit efforts as requested Keep current on changes and expectations as they are delivered through team communication.
Engage in department, team, and all staff meetings Actively participate in agency training and take the initiative to use information obtained in training in your work Follow all PCS policies and procedures and keep current on all required training Safeguard the security and confidentiality of protected health information in accordance with PCS policies Demonstrate ability to use and follow best practices for completing job requirements Complete all applicable deadlines in accordance with the PCS procedures for the Support Coordination team Work with your supervisor on benchmarks and make improvements where needed MISSION STATEMENT TO BUILD RELATIONSHIPS THAT WILL INSPIRE POSITIVE SOCIAL CHANGE, EMPOWER PEOPLE AND CHALLENGE THE STATUS QUO.
